Not necessarily. Unless it is already a multiple book deal, then this is very rare. With the speed of Jack's writing, Beth Meacham must have seen a reasonable advanced plan chapter/plan to have sanctioned the deal. After all, Jack unfortunately, is not one of these million-selling hack-and-slash two-dimensional (sorry about all the hyphens!!) fantasists that get most of publishers money these days.

Again, not necessarily. It depends on the author and the deal. Several authors have sold sequels to different publishers. Terry Pratchett is one that springs to mind. You're correct about the second point but as I say above, his editor must have seen a decent chunk.
